Fixed bugs which prevented setting new defaults on update and when resetting to default settings.
Made it easier to re-order options via drag and drop.
Added option to hide 'Use Alternate' menu item, so now if only one option is enabled it is not placed in a submenu.
Added support for submitting non-urlencoded urls.
